E10 J can chir, Vol. 52, N 1, février 2009 © 2009 Association médicale canadienne I ntra-articular fractures of the metacarpal head are rare, often requiring surgical fixation to maintain articular congruence. McElfresh and Dobyns presented a 10-group classification of 103 such fractures based on anatomic involvement and fracture pattern (Table 1). They identified only 4 with a horizontal fracture pattern. On a further review of the literature, Hastings and Carroll identified 4 additional cases with this pattern. Surgical management, long-term outcome and the possibility of extrusion of the metacarpophalangeal joint capsule were not described. We report the unusual case of a horizontal fracture of the metacarpal head, complicated by 90° dorsal rotation about a transverse axis, and extrusion of the head fragment from the joint capsule. We describe our approach to assessment, surgical management and successful restoration of function.

Treatment of horizontal root fracture with extrusion: A case report
Root fractures are defined as fractures involving dentine, cementum, and pulpal and supportive tissues. They comprise about 0.5–7% of all dental injuries. Horizontal root fractures are commonly observed in the maxillary anterior region and 75% of these fractures occur in the maxillary central incisors. Case report: This case report is about a 26-year-old male referred to the endodontic departme...
متن کاملMetacarpal Fractures
The majority of metacarpal fractures are closed injuries amenable to conservative treatment with external immobilization and subsequent rehabilitation. Internal fixation is favored for unstable fracture patterns and patients who require early motion. Percutaneous pinning usually is successful for metacarpal neck fractures and comminuted head fractures. Shaft and base fractures can be treated wi...
متن کاملFractures of the proximal phalanx and metacarpals in the hand: preferred methods of stabilization.
Treatment of fractures of the proximal phalanx and metacarpals is based on the presentation of the fracture, degree of displacement, and difficulty in maintaining fracture reduction. A wide array of treatment options exists for the variation in fracture patterns observed. Inherently stable fractures do not require surgical treatment; all other fractures should be considered for additional stabi...
متن کاملSmall Finger Metacarpal Shaft Wedged between Ring and Small Metacarpal Bases: a Report of 2 Cases
We present two patients with a displaced fracture of the small finger metacarpal base, where the shaft of the small finger metacarpal was wedged between the bases of the ring and small finger metacarpals. The striking appearance on radiographs led to initial recommendations for surgery, but both patients preferred non-operative treatment and did well in the short term without surgery
متن کاملA Case of Fifth Metacarpal Neck Fracture
Aims: Determining the impact mechanism is one of the most important measures in forensic science. The mechanism of fracture of the metacarpal neck is squeezed directly into the fist. Often, this type of fracture is due to the impact of the first depending on the hard. In this study, we introduce a case of this fracture. Patient’s Information: He was a middle-aged man, who had ache with various ...
